§ Support the child/youth in achieving their identified goals. § Identify any barriers to achieving these goals and resources required to resolve problems. § Meet fortnightly for one on one with the child/youth to review goal progress. § Communicate to parents the progress of the child/youth in the Residential setting. § Co-ordinate and ensure that the needs and interventions of the child/youth in the Residential Service are met. § Participate and contribute in meetings regarding the needs of the child/youth.
Your child has been referred to Westbridge to focus on his/her target behaviours and learn new skills to utilise when they return to their mainstream school and wider community. When your child returns home it gives both the family and child the opportunity to practise the changes that have been put in place. The staff here will work with you to return your child back to his/her mainstream school and community, therefore the family will need to be part of the programme.
